Build from a Declaration of Intent Released by Signify’s Board of Directors in late 2021, the report provides a framework for how the company will measure its performance on key environmental, social and corporate governance issues, in addition to healthcare quality metrics , in accordance with its vision and values.

The report includes key achievements for 2021 and establishes a baseline against which the company will measure progress in several focus areas. Among them:

  • Lead with quality. Signify conducts evidence-based evaluations to help providers achieve quality outcomes, and in 2021 exceeded national averages for several quality measures under the Advanced Bundled Payments for Care Improvement Program ( BPCI-A) from Medicare. These quality measures include: the AHRQ Patient Safety Indicator (PSI 90); advance care planning; Excessive days in acute care after hospitalization for acute myocardial infarction; and Perioperative care: selection of prophylactic antibiotics. Additionally, the company reported double-digit percentage reductions in 90-day hospital readmissions among BPCI-A and ACO patients participating in its Home Transition Program. Also in 2021, the National Quality Assurance Committee (NCQA) validated the methodology used by Signify to identify eligible members to receive specific diagnostic and preventive services during a Signify in-home assessment.
  • Management of Social Determinants of Health (SDH). Signify’s more than 600 social care coordinators screen individuals for social issues that affect their ability to access quality health care and achieve the best possible health outcome. Through these efforts, in 2021, Signify connected more than 260,000 people to services to address social determinants of health (SDH) issues such as food insecurity, social isolation, and transportation.
  • Building sustainable workplaces. To reduce energy consumption and emissions, the company reported several initiatives to incorporate environmentally friendly building materials into its offices in Dallas, Oklahoma City and Rapid City.
